Waiver and Release of Liability:

In consideration of being allowed to participate in any way in the programs, related events, and activities of Champion Taekwondo Academy, I, {name}, acknowledge, appreciate, and agree that:

1. Risk of Injury: The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death. While particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.

2. Fit for Participation: I confirm that I am willing and able to engage in the activities involved in this program. I acknowledge that I am responsible for my own safety and well-being.

3. Release: I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation.

4. Consent: I willingly agree to comply with the stated and customary terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

5. Medical Authorization: In case of an injury, accident, or illness, I authorize Champion Taekwondo Academy to obtain necessary medical treatment for me and hereby, in my name, authorize any licensed physician and/or medical facility to provide medical assistance deemed necessary. I agree to bear any costs incurred as a result of such treatment.

6. Acknowledgment of Understanding: I have read this waiver and release of liability and fully understand its terms. I acknowledge that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to provide a complete and unconditional release of all liability to the greatest extent allowed by law.

 Class Time: Based on your registration, your class time is set as follows:

Tiger Tots (Ages 4 - 6):

Saturday Class 1: 9:30 AM - 10:15 AM
Saturday Class 2: 10:15 AM - 11:00 AM

Little Tigers (Ages 6–10):

Monday: 6:00 PM - 7:00 PM
Wednesday: 5:30 PM - 6:15 PM
Friday: 6:00 PM - 7:00 PM
Saturday: 11:00 AM - 12:00 PM

Big Cats (Ages 11–16):

Wednesday: 6:15 PM - 7:00 PM
Saturday: 12:00 PM - 1:15 PM

Adult Taekwondo:

Monday - Wednesday - Friday : 7:00 PM – 8:15 PM 

Monthly Payment Information: A reminder that the monthly class fee is $100 per student. Payments are due by the 5th of every month.
